Not related to hormones. It is simply due to the extra fat accumulation in the chest area, usually caused by weight and/or fat gain.

The affected area can feel sore, tender, and/or painful, accompanied with swelling and sometimes, nipple discharge. You will feel a firm, rubbery lump in one or both breasts.
✕ Pseudogynecomastia
Breasts feel soft and evenly enlarged. There is no pain or soreness upon touch. Your areolas are not enlarged, and your nipples are not sore or discharging.
The initial stage is characterised by minor breast enlargement without the development of excess skin.
The excess skin, at this stage is found around the areola which is the coloured tissue that is surrounding the nipple.
As the gynecomastia condition enters into the next stage, there will be a development of moderate breast enlargement without the development of excess skin.
However, with enlarged male breast tissue, it now extends beyond the areola, with a minor degree of enlargement.
At this stage, there will be moderate breast enlargement with the development of excess skin.
As the enlarged male breast tissue extends further into the breast zone, the male chest starts to look slightly like the feminine breasts and at this stage, the condition becomes clearly noticeable by others.
When you pinch typical breast tissue, it usually feels firm. The tissue may also resist the pinch a little. Also, when you pinch the areola from end to end with your thumb and forefinger, the tissue rim should be no bigger than 1/2 cm in diameter. When one has gynecomastia, there would be no resistance and no tissue rim.

Gynecomastia can be caused by hormonal irregularities, certain medical conditions, certain medications, such as anti-androgens, anti-anxiety or antidepressant medicines, and anabolic steroid for bodybuilding, excessive consumption of alcohol. In some cases, the causes of gynecomastia may be due to genetics or simply, idiopathic.
Gynecomastia, in general, is classified into:
• True gynecomastia (where glandular tissue is the only cause)
• Mixed gynecomastia (where it involves both glandular and excess fat tissues. This is also most commonly seen)
• Pseudogynecomastia (where it involves only the proliferation of chest fat tissues)
True and mixed gynecomastia are divided into four different "grades," based on the severity of the problem.
During the clinical assessment, Dr. Ivan Puah will assess the breast size, degree of breasts droopiness, the presence of excess skin, glandular tissues and nipple puffiness, your skin elasticity and chest shape. He will also discuss with you on his treatment approach and plan after the assessment is done.
Unfortunately, it is impossible to reduce overly enlarged male breasts with diet and exercise. In some cases, excess chest fat may respond to diet and excuse, but glandular tissues do not. This male breast disorder will require surgery to address the issue effectively.
Despite what's being purported, there is no scientific evidence to prove that topical creams, non-surgical fat reduction machines such as fat freezing, or massage techniques can solve the condition.
